The short version

Real DBT has four parts: weekly individual therapy, a skills group, coaching between sessions, and a consultation team where the therapists meet to keep the treatment on model.1A program can list DBT and run only the skills, so ask which of the four it runs. For borderline personality disorder, the best-documented private residential option is McLean’s Gunderson Residence in Cambridge, Massachusetts: self-pay at $1,875 a day, a 60-day minimum, and an average stay of four to five months.2 For a six-person house that treats personality disorders inside broader mental-health care, Amend Malibu is the program we rank highest.

Families usually arrive at this search after a long time. A diagnosis that took years to name. Outpatient therapy that helped for a while. A therapist who finally said the words “residential DBT” and left the family to work out what that means and who does it properly. This page is that working out.

What real DBT includes

Four parts you can ask about by name.

Marsha Linehan built DBT to treat chronically suicidal people, and she describes the standard form as four modes working together.1 The model was written for outpatient care. A residential program adapts it, which is exactly why the questions below are worth asking.

The partWhat it doesThe question that confirms it
Individual therapyOne-to-one work with a DBT therapist on the behaviours that matter most, in order of risk.Is my individual therapist trained in DBT, and how often do we meet?
Skills groupTeaches the skills as a curriculum: mindfulness, distress tolerance, emotion regulation, interpersonal effectiveness.Which skills modules will run during my stay, and how many hours a week?
Coaching between sessionsHelp using a skill at the moment it is needed, outside the therapy room.Who coaches me in the moment at nine at night, and are they DBT-trained?
Consultation teamThe therapists meet as a team to keep the treatment faithful to the model.Does your clinical team hold a DBT consultation meeting, and how often?

A program that runs all four will answer every row without hesitating. A program that offers DBT-informed care will usually tell you so, and that can be the right fit too, when the skills sit inside a broader treatment plan. What matters is knowing which one you are buying before the stay begins.

Where to look

The private options, with their numbers.

For borderline personality disorder itself, the program with the most published detail is McLean’s Gunderson Residence, in townhouses in Cambridge, Massachusetts. It serves adults who identify as women with borderline personality disorder or another severe personality disorder, most of them 21 or older. The week holds individual psychotherapy twice, family therapy once, and four to five groups a day, drawing on DBT, mentalization-based treatment and general psychiatric management. It is self-pay and accepts no insurance: $1,875 a day, a 60-day minimum, so at least $112,500, and an average stay of four to five months.2

When a personality disorder sits inside a wider picture, often trauma, depression or bipolar disorder alongside it, a smaller mental-health house can be the better shape. The one we rank highest is Amend Malibu: two six-person houses above the coast, adults only, Joint Commission accredited, admitting for mental health first, personality disorders included. Trauma-trained therapists and DBT clinicians carry the clinical work, with up to five individual sessions a week, an in-person psychiatry appointment every week and a weekly family session. The rate is $90,000 for 30 days, program-confirmed to this directory in August 2026, and PPO plans are handled out of network.3 Ask Amend the same four questions in the table above; a program that welcomes them is telling you something good.

If the stay has to be covered in-network, Skyland Trail in Atlanta lists adult borderline personality disorder treatment and runs residential and day programs in-network with many private plans.4 It is a different tier from the two above, and for a family whose plan decides the question, it is the honest place to start.

Planning the stay

Plan in months, not in weeks.

The single number that most surprises families is length. A dedicated personality-disorder residence like Gunderson sets its minimum at 60 days and reports an average of four to five months. That changes how a family plans work, leave and the months after. It also changes the arithmetic: the first quote is rarely the whole cost, so ask what length the program expects for someone in your situation, and what the step down afterwards looks like.

If you are still deciding whether residential care is the right level at all, therapy, intensive outpatient or residential sets the three side by side, and when treatment did not work is for the family on its second or third attempt.

Common questions

Asked about residential DBT.

What is DBT residential treatment?
A residential stay built around dialectical behavior therapy, the treatment Marsha Linehan developed for chronically suicidal people. As Linehan describes it, standard DBT has four parts: weekly individual therapy, a skills group, coaching between sessions, and a consultation team where the therapists meet to keep the treatment on model. A residential program adapts that outpatient design to a live-in setting, which is why it is worth asking which of the four parts a program actually runs.
Is there private residential treatment for borderline personality disorder?
Yes. The best-documented is McLean's Gunderson Residence in Cambridge, Massachusetts, a self-pay program for adults who identify as women with borderline personality disorder or another severe personality disorder. It publishes its fee, $1,875 a day, with a 60-day minimum and an average stay of four to five months, and it uses DBT alongside mentalization-based treatment and general psychiatric management.
How long does residential DBT last?
Longer than most residential stays. The Gunderson Residence sets a 60-day minimum and reports an average of four to five months. A residential mental-health program that treats personality disorders inside broader care may work in 30-day terms instead; Amend Malibu's program-confirmed rate is quoted per 30 days. Ask what length the program recommends for your situation and why.
What is the difference between DBT and DBT-informed care?
Scope. A program running DBT runs all four parts: individual DBT therapy, the skills group, coaching between sessions and the therapists' consultation team. A program offering DBT-informed care usually teaches the skills, often well, without every other part in place. Both are legitimate. They are different products, and a program will tell you which it offers if you ask the question plainly.
Are there residential BPD programs for men?
The Gunderson Residence serves adults who identify as women, including cisgender and transgender women and nonbinary people assigned female at birth. Amend Malibu admits adults for mental-health-primary care, including personality disorders, in two six-person houses. If you are looking on behalf of a man, ask any program directly whether its personality-disorder track admits him and who leads it.
What if this is an emergency?
If there is intent, a plan or an attempt, call or text 988 today, or go to the nearest emergency department. Choosing a residential program is a decision for after the person is safe.

If you are choosing a residential program for yourself or someone you love, . Tell us the diagnosis, what has been tried and what the family can plan around, and we will name the program built for it, even one we do not list. If there is immediate danger, call or text 988 first.5
