Aftercare

Also called: continuing care, step-down care.

Short definition

Aftercare is the ongoing clinical support after leaving a residential program — therapy, group meetings, family work, and structured living during the high-risk months that follow.

The months right after leaving residential treatment are the highest-risk period for relapse. Aftercare addresses this head-on. A good aftercare plan typically includes outpatient therapy matched to the client's stability, group support (twelve-step, SMART, or alumni groups), family work where needed, and structured living arrangements.

For luxury residential alumni, aftercare often runs through the program's own outpatient arm — staying with the same clinical team has real advantages over being handed off to a separate provider at discharge. Programs that maintain involvement after residential produce better outcomes than those that don't.

The question to ask before admission is not whether the program has an aftercare plan, but whether they have an aftercare team — dedicated staff whose job is to manage the transition from residential back into the world.
