The price

The room sets the price: $49,000 to $69,000.

Exclusive Hawaii Rehab publishes one price for the 30-day program and varies it by room.1

  • $49,000: a semi-private room with a shared bath.
  • $59,000: a private room with a shared bath.
  • $69,000: a private room with a private bath.

Meals are chef-prepared, and detox happens on site: the program’s medical director sets a detox protocol from blood work taken on arrival, which is why admissions run Monday to Friday, with arrival between 9 AM and 1 PM.3

Longer stays

A second month costs either another block or about $15,000.

The program is built as a 30-day stay, and clients stay 30, 60 or 90 days.1 There are two ways to stay on. You can buy another 30 days of the program, or move into extended-care housing at about $15,000 per 30 days, which includes daily treatment at the main facility.

So a 60-day stay comes to about $64,000 to $84,000 with a month of extended care after the first. If you would rather stay in the main program for the second month, ask admissions for that price in writing before you choose a room.

Insurance

You pay first, and your insurer reimburses you after.

Exclusive Hawaii Rehab accepts out-of-network and private-pay insurance from most major insurers. The program asks for payment up front. After you complete it, your insurer processes the claim and reimburses you according to your benefits for residential addiction treatment.2

That makes two numbers matter: the full price, which has to be available on admission, and your plan’s out-of-network benefit for residential care, which decides how much comes back. The program has an insurance specialist who checks benefits before you book. Get the answer in writing.

The program

A small program on an ocean cliff at Hakalau.

The Exclusive sits on a 30-acre private oceanfront property on the cliffs of the Hamakua Coast, north of Hilo on the Big Island. It describes itself as an eight-bed facility with private and shared rooms.3 The state’s accreditation list, revised 24 June 2026, records it as an adult residential program with 14 beds, not state-contracted.4

It holds both credentials Hawaii issues. The Department of Health licenses it as a Special Treatment Facility,3 and the Alcohol and Drug Abuse Division’s accreditation is dated 1 June 2026, running to 31 May 2027.4 The program also reports a three-year CARF accreditation.3

Hilo International Airport is 35 to 37 minutes away by car, and Kona International about an hour and 45 minutes, measured on Google Maps in September 2026.5

At the same price

The alternative at this price is on Maui.

Maui Recovery in Lahaina is program-confirmed from $60,000 for 30 days as of August 2026, takes insurance out of network after verifying the policy, and keeps eight guests across three houses, each with a private room.6789 It also lists PTSD, depression and anxiety among the conditions it treats.10

The practical difference is detox. The Exclusive runs it on site. Maui Recovery admits once withdrawal management is complete, at Aloha House on Maui or on the mainland (operator-confirmed, 31 July 2026). If you need detox as part of the stay, that settles a lot. If you do not, the choice comes down to island, room and which program’s approach fits.
