What works
Three therapies have the strongest evidence.
PTSD is common. About 6 in every 100 people will have it at some point in their lives, and about 5 in every 100 adults have it in any given year.1 It is also treatable, and the treatments with the most research behind them are specific enough to ask for by name.
Trauma-focused talk therapy works on the traumatic event directly and on what it has come to mean to you. The National Center for PTSD names three that work best2:
- Cognitive Processing Therapy (CPT). You examine and change the beliefs the trauma left behind, such as blame, shame or a sense that danger is everywhere.
- Prolonged Exposure (PE). You gradually approach the memories and situations you have been avoiding, until they lose their hold.
- EMDR. Eye Movement Desensitization and Reprocessing: you recall the memory while following a back-and-forth movement or sound, which helps the mind process it.
Ask any program which of the three it delivers, who delivers it, and how many sessions a week you would have. A therapy listed on a website and a therapy delivered twice a week by an experienced clinician are different things, and good programs answer the question easily.
The levels
Weekly outpatient therapy is the usual starting point.
Weekly therapy with a trauma-trained clinician is the right level for most people with PTSD, and it is the level Hawaii has most of. Honolulu has the widest choice of specialists. The neighbor islands have fewer, and telehealth has closed some of that gap. SAMHSA’s findtreatment.gov searches licensed options by island and level of care.11
Residential treatment earns its place when daily life has stopped working, when drinking or drug use has become tangled up with the trauma, when home keeps setting off the symptoms, or when outpatient therapy has been tried and has not been enough. Veterans can start with the VA Pacific Islands Health Care System, which offers PTSD assessment, private counseling, group therapy and medication. Every VA facility offers same-day mental health help, and you may qualify even without enrolling in VA health care.9
