You found a program that looks right, you have Medi-Cal through Kaiser or straight Medicaid or Medicare, and the admissions coordinator says, kindly, that the program works with private insurance and direct payment only. Nobody on that call did anything wrong. But nobody handed you a map, either.

  • Your state and county behavioral health agencies. Every state runs an agency that administers public treatment dollars, and FindTreatment.gov lists each state’s contacts. In California, county behavioral health departments run public mental health care for Medi-Cal members, so the county line is the right first call there. Calling the agency is not a last resort; it is asking the people who fund the system for directions through it.
  • Community mental health centers. The workhorses of public mental health care. They see people on Medicaid and Medicare, many work on a sliding scale when coverage is thin, and they can evaluate whether a residential stay is the right next step or whether intensive outpatient work, several sessions a week while you live at home, gets there without one. Call the behavioral health or psychiatry department of the nearest large hospital and ask which programs they run and whether they take your plan; the program checks what your insurance will pay before anything is scheduled.   • The VA, if you served. The VA runs mental health residential rehabilitation treatment programs, live-in care for eligible veterans, alongside its outpatient services. Start at va.gov’s mental health pages or ask at your local VA medical center.

In order, starting tonight.

  1. If safety is in question, call or text 988 now, or go to the emergency department. Everything else on this list can wait a day.
  2. Open FindTreatment.gov. ZIP code, payment filter, Medicaid or Medicare. Write down three programs.
  3. Call the member services number on your insurance card and ask for the behavioral health line. Ask which residential or inpatient mental health programs your plan covers and how a referral works.
  4. Call your county behavioral health department and ask the same question. In California, the county runs Medi-Cal mental health care.
  5. If you served, ask the VA about its mental health residential rehabilitation programs.
  6. Call two programs from your list and ask three things: is there a bed, how long is the wait, and what do you need from me.
