In short

Oahu holds the widest range of treatment in Hawaii: hospital psychiatric care, nonprofit and state residential programs, outpatient clinics and detox, nearly all in and around Honolulu. Hawaii CARES answers at 988, 24 hours a day. For private residential care, Maui Recovery in Lahaina is a half-hour flight away.

Most treatment in Hawaii sits on Oahu, because most of Hawaii sits on Oahu.

The island carries hospital psychiatric units, community and nonprofit residential programs, outpatient clinics, methadone and buprenorphine services, and the state’s central access route. For someone who needs care inside the public system or inside a network, this is where the options are, and findtreatment.gov searches them by level of care and location2.

Hawaii CARES is the fastest way in. It answers 24 hours a day at 988, covers the whole state, and can route someone into assessment and placement in a single call3.

Sorting the levels

Four levels, and what each one is for.

Detox handles withdrawal, usually three to seven days, and alcohol and benzodiazepines are the two that need medical supervision without exception.

Residential treatment is the month or more that follows, where the whole day is built around recovery. Intensive outpatient runs several sessions a week around work and family. Standard outpatient is the weekly appointment that carries most people most of the time.

The order matters more than the brand of any one program. Detox that ends without a residential or outpatient placement already booked is where a lot of recoveries are lost, so ask any provider on Oahu what the handover looks like before the admission, not after.

The interisland option

Twenty-five minutes changes the field.

A flight from Honolulu to Kahului takes about half an hour, which puts Maui’s private residential programs inside easy reach of anyone on Oahu.

Maui Recovery sits on twenty private acres in Lahaina, a mile from the water, treating alcohol and opioid use, trauma and PTSD, dual diagnosis, depression, behavioural addictions and executive burnout. It is accredited by the Joint Commission and a member of NAATP14, and the rate is program-confirmed with us from $60,000 for 30 days as of August 2026.

For an Oahu resident there is a second argument for the flight, and it is clinical rather than scenic: distance from the people, streets and routines attached to the drinking or the using. On an island where everyone has a cousin in common, that distance is worth something real.
