The private tier

Two private routes: detox on site, or detox and then Maui.

Detox and treatment in one stay, on the Big Island. Exclusive Hawaii Rehab at Hakalau, on the Hamakua Coast north of Hilo, runs detox inside its 30-day program. Its medical director sets the detox protocol from blood work taken on arrival, which is why arrivals run Monday to Friday between 9 AM and 1 PM. Detox first, then a month on Maui. Maui Recovery in Lahaina does not run detox. Detox happens first, on the mainland or on Maui, and Maui Recovery admits once it is complete (operator-confirmed, 31 July 2026). It keeps eight guests across three houses above Lahaina.4 A 30-day stay is program-confirmed from $60,000 as of August 2026, paid privately or through PPO out-of-network benefits checked with admissions.23 Kahului Airport is 36 to 43 minutes away by car.5

Through insurance or Med-QUEST

In-network and Med-QUEST detox is a different kind of program.

These programs are larger, contracted by the state or in-network with insurers, and paid through insurance or Med-QUEST rather than privately. For many people they are the right route. Hawaii CARES, at 988 or 1-800-753-6879, connects people anywhere in the state to them and finds an open bed.9

  • Maui: Aloha House in Makawao, where most people stay five to seven days with nursing around the clock; it takes many forms of insurance and helps with Med-QUEST enrollment.67
  • Oahu: the Salvation Army’s Addiction Treatment Services for men and its Women’s Way program for women, and the Institute for Human Services’ Imi Ola Piha.7
  • Big Island: a men’s unit at the Big Island Substance Abuse Council, under state contract,7 and Hawaii Island Recovery in Kailua-Kona, which publishes a medically assisted detox program and is in-network with most major insurers.8
